Monogram Health delivers in-home nephrology and multispecialty care, along with benefit and case management services, for patients with chronic and end-stage kidney disease and related chronic conditions. It contracts on a value-based basis with health plans and risk-bearing providers such as Humana and Aetna, and it operates in about 36 states. It was founded in 2019, and a $375M Series C closed in January 2023, bringing total funding to roughly $550M. Backers include TPG, Norwest, Frist Cressey Ventures, CVS Health Ventures and Cigna Ventures. Inc. ranked it third on its 2024 Inc. 5000 list and described it as a business of more than $2B.
