Aledade partners with independent primary care physicians and community health centers to build and run Accountable Care Organizations, offering practice transformation services, upfront capital, data analytics and a technology platform. It was founded in 2014 by Farzad Mostashari, Mat Kendall and Edwin Miller, and raised a $260M Series F in 2023 led by Lightspeed Venture Partners at a $3.5B valuation. It also secured $500M in debt financing in December 2025, and the company reports 3K+ primary care partners across 46 states and D.C.
